  2. If you are sharing documents to a Shared Care Record – what applications are used to provide the documents?

  4. Is there an information sharing agreement in place that covers all document sharing scenarios and use cases with the Shared Care Record, or is Information Governance approval required to increase the scope of what is shared each time?

Yes, The North Central London/onelondon Record London Care Record (LCR)

The London Care Record system software

  1. Do you share all documents with the Shared Care Record, or a subset? If a subset can you advise what the key documents are that are ingested into the shared care record?

Details on the documentation which may be captured/shared within the LCR are publicly available on the LCR website, which may be reached via the following link The London Care Record | North Central London Integrated Care System.

Yes

No

GP/Care Plan letters, Patient demographic information (name/address/GP etc),

Referral Information, appointment details, diagnostic information.

All healthcare organisations within the NCL ICB catchment area.

  1. Do you pay an annual subscription or a one-off fee to support document sharing into the shared care record?

No.  The Trust does not make any financial contributions for use/access of the LCR system.

  1. When does your existing Shared Care Record contract come to an end?

The Trust does not hold the contract for this and does not hold any data regarding its renewal or termination.  It is provided to all NHS healthcare organisations within the NCL ICB catchment area, free of charge.

  1. If you are only sharing a subset of documents, are you planning to extend this subset to provide additional document access?

The Trust does not hold this contract, as explained above.

Yes, subject to provision of relevant access controls and permissions

All types of documents, subject to their availability on the console.
