I am writing to make a request under the Freedom of Information Act for the Trust’s official policy for

- recording of sexual safety incidents, including sexual abuse, sexual harassment or sexual assault through the National Reporting and Learning System (NRLS) or any other relevant system.
At the Tavistock and Portman NHS Foundation Trust we have an Incident Reporting Procedure, which is attached.
There is also a monthly Incident Panel to review these incidents, and a specialist team for reporting Patient Safey Incidents to external authorities, previously NRLS and now PSIRF. Serious Incidents are also reported to the StEIS portal.
We have a Safeguarding lead for Children and Young Adults and Safeguarding lead for Adults, and we also we have a Caldicott Guardian, all of whom may be approached.
All staff are trained on reporting Incidents as part of their Induction training and all training materials and guidance are on our Intranet. Safeguarding Training and the reporting of Safeguarding and Serious Incidents is also entered on our Risk Register and monitored for mitigating actions.
