The Trust’s Patient Safety Incident Response Framework (PSIRF) Plan is attached for your perusal. As you may be already aware, PSIRF is mandated by NHSE and its introduction will set out an improved way for staff across the Trust to report and learn from Patient Safety Incidents. Please kindly note that the attached PSIRF plan is an evolving document, and will therefore be continually subject to changes up to and beyond the point of PSIRF implementation.
