I would be grateful if you could provide the following information for Tavistock and Portman NHS Foundation Trust for the most recent 12-month period available:
This request is for aggregate, anonymised data only. No personally identifiable information about patients or staff is being requested.
- The number of serious incidents (as defined by your Trust and national reporting requirements) in which a doctor was directly involved in the patient’s care.
- Where available, a breakdown of these incidents by:
- Incident category (e.g. patient suicide, homicide, other serious or traumatic events), and
- Medical grade (e.g. Consultant, SAS doctor, Resident doctor).
Response sent
The Trust no longer uses the terminology ‘Serious Incident’ or ‘SI’. Instead, we now refer to ‘Patient Safety Incident Investigations’ (PSIIs), in accordance with the Trust’s Patient Safety Incident Response Framework (PSIRF).
- The number of serious incidents (as defined by your Trust and national reporting requirements) in which a doctor was directly involved in the patient’s care.
The Trust confirms that this was zero for the period January 2025 to January 2026,
- Where available, a breakdown of these incidents by:
- Incident category (e.g. patient suicide, homicide, other serious or traumatic events), and
- Medical grade (e.g. Consultant, SAS doctor, Resident doctor).
