Under FOI, I request sight of any guidance that your ‘panel’ are required to follow where record-keeping and decision-making are concerned.

I do not accept [name withheld] assertion that the panel create no records – this surely cannot be in accordance with NHS policy.

We understand that your questions relate to the Psychotherapy Team Treatment Panel:

  1. The remit of this panel is to review the outcome of assessments and confirm if treatment will be offered as individual once weekly psychotherapy, or group psychotherapy
  2. The panel currently meets weekly due to high demand, but otherwise this would be fortnightly.
  3. It is attended by two Consultant Psychiatrists, and two Highly Specialist Psychotherapists.
  4. The panel does not have Terms of Reference.
  5. Only the outcome of the panel discussions is noted, and this is entered into the patients’ electronic patient record (EPR).

No minutes are kept of the whole panel session in single document format
