  1. Please disclose since 1994, how many homicides by trust patients have occurred within a year of contact with the trust.

a. How many of these homicides by trust patients were reported to NHS England or its predecessor bodies?

b. Please give a breakdown by year.

2. How many of the total number of homicides by trust patients reported to NHS England were independently investigated under HSG 94-27?

3. If not all homicides by trust patients were reported to NHS England and/or its predecessor bodies, can the trust explain why this was so?

Please note that the requested data, presented in two data tables below, involves low numbers, i.e. equal to or less than five, which have masked as the symbol ‘≤5’. Were we to disclose the data requested it could potentially lead to identification or re-identification of the individual(s) concerned.  Although the year alone is not personal data, we have also to consider whether other information that is already available, or may become available, to any member of the public, could be combined with the data requested so as to enable identification of the individual(s) concerned. We are, therefore, withholding this information under S.40(2) Personal Information.
