Details of enquiry

Between 01/01/2014 (or the start of operations for the service, if later) and todays date;

1. The number of Activity Query Notices (AQN’s) the trust has sent to NHS England (NHSE) related to the gender dysphoria service the trust runs,
2. A copy of any AQN’s or AQN Follow ups the trust has sent to NHSE related to the gender dysphoria service the trust runs,
3. A copy of any replies the trust received from NHSE regarding the AQN’s mentioned above,
4. The dates of any Activity Management Meetings, utilisation meetings or joint activity reviews held with NHSE secondary to an AQN as described above being issued.

Please note that for the (Adults) Gender Identity Clinic, we do not hold data prior to 1 April 2017, which was when the service transferred from the West London NHS Mental Health Trust to the Tavistock & Portman NHS Foundation Trust.
