  1. The trust supplied the Case Notes Review, but what did Professor Appleby actually ask for?  Please can you provide me with a copy of that data request?
  2. Please can you provide me a copy of anything the Trust supplied alongside the Case Notes Review to give context specifically about the limitations and completeness of the data?
  3. Was there any other communication with Professor Appleby (before or after the publication of his report) in relation to the limitations and quality of the data?  Please provide me with copies of any related correspondence

  1. The trust supplied the Case Notes Review, but what did Professor Appleby actually ask for?  Please can you provide me with a copy of that data request?

The Trust cannot answer this question, because it does not hold the data you request.

The Trust confirms that there was no liaison between Professor Appleby and the Trust prior to or during his Review of Suicides and Gender Dysphoria at the Trust, which was commissioned by NHS England and not by the Trust.

This question would better be addressed to NHSE who commissioned the Review. Their FOI team may be reached via england.contactus@nhs.net

The Tavistock and Portman NHS Foundation Trust confirms that it did not liaise with Professor Appleby nor his team, and did not commission nor have any dealings with the work of the review
